I'm not quite ready to throw the baby out with the bath water, but that time might be coming. Tom Coens and Mary Jenkins reach a similar conclusion in Abolishing Performance Appraisals. I contend that the issue isn't the concept of performance reviews, or often even the forms or process. It's that managers don't do their jobs. The jobs of setting good goals, giving periodic feedback and being honest about performance. If these things would happen, the rest is gravy. I write about performance often in my blog - www.hrguy4omni.wordpress.com.
